Planning Board Forms/Submittal Requirements

The following forms are used as part of the submission process for subdivisions, site plans, or rezoning applications in the Town of Ithaca.  In most cases, the original signed forms, along with additional relevant application materials (e.g. survey and subdivision plats, site plans, detail plans, traffic analysis, etc.), should be submitted to the Planning Department at least 30 days prior to a scheduled Planning Board meeting.

Below is an explanation of when to use each submission form.  The various forms can be picked up at Town Hall or printed by clicking on the links associated with each form below.

Please contact the Planning Department (273-1747) to set up a pre-application meeting with one of the staff members before filling out or submitting any forms.



Development Review Application Form - This form should be submitted to the Planning Department for all subdivision, site plan, special permit, or rezoning requests.  The required fees associated with each action are listed on the back of the form.  Planning staff can assist with determining the appropriate fees for a project.  Applicants should submit one original signed Development Review Application form and the fee payment, along with the other required application materials. Note: A new application form is required to be filled out for each stage of a project review.




 


Environmental Assessment Forms:         
Short Environmental Assessment Form
- This form is used for all small subdivisions, site plans, or rezoning submissions that are classified as an “Unlisted” action, according to the State Environmental Quality Review Act or SEQR (6 NYCRR Part 617).




 

Full Environmental Assessment Form

This form is used for larger projects that may create  more significant impacts.  It is optional for “Unlisted” actions (determined by Planning staff at a pre-application meeting) but is required for “Type I” actions.  The SEQR regulation lists the actions that are considered “Type I”.  This listing can be found on the NYS DEC website at: http://www.dec.ny.gov/regs/4490.html#18106.The Town of Ithaca also has its own Environmental Review Law (Local Law No. 5, 1988), which includes a list of locally-identified Type I actions.


Owners Certification Letter- This form only applies to subdivision submissions.  It should be filled out by all current owners of the property (no picture).



Agricultural Data Statement- This form applies to properties that lie within one of the Tompkins County Agricultural Districts.  In the Town of Ithaca, this includes areas on West Hill and a section in the Southeast corner of the Town.  You can find a map of the Agricultural Districts on the Town Agriculture page.



Checklists– The Sketch, Preliminary and Final Site Plan checklists and the Preliminary and Final Subdivision checklists are provided to applicants so they can see what the Town considers as complete application information.  Planning staff will use these checklists to determine application completeness for each project that appears before the Planning Board.  Some elements can be waived for smaller projects, but all elements are generally discussed with Planning staff at a pre-application meeting.
